Pharmacy
The Gundersen Lutheran Health Plan Pharmacy Department oversees the operation and administration of the prescription drug benefit program for our members. This includes developing policies and procedures that meet all federal, state and the National Committee for Quality Assurance (NCQA) standards and regulatory requirements.

The department also monitors formulary compliance, coordinates prescription authorization activities, and monitors utilization of pharmacy services. In addition, the Pharmacy Department analyzes new pharmaceuticals and oversees the drug formulary.

The Drug Formulary document is developed by the Gundersen Lutheran Health Plan Pharmacy and Therapeutics Committee (P&T Committee). This committee includes at least one practicing pharmacist and one practicing physician who are experts in the care of elderly or disabled persons. At least one P&T committee member is a practicing pharmacist and one practicing physician, who is independent and free of conflict with respect to our Health Plan, our Part D plan and/or pharmaceutical manufacturers.

Pharmacy Management inquiries should be directed to the Health Plan at (800) 897-1923 extension 58077 or 608-775-8077.
