BadgerCare Plus is a program for children under 19 years of age and families in Wisconsin who need and want health insurance. BadgerCare Plus is for all kids, regardless of income. BadgerCare Plus also offers access to comprehensive, affordable health care for many families and pregnant women in Wisconsin. To learn more, visit the ForwardHealth Web site.

BadgerCare Plus has three benefit plans:

- CORE Plan (for adults with no dependent children)

The plan in which you are enrolled depends on your income. Eligibility for BadgerCare Plus is determined by the State of Wisconsin Department of Health Services and it's easy to complete an on-line application at access.wisconsin.gov.
Pharmacy, Chiropractic and Dental coverage for BadgerCare Plus members is provided by the State of Wisconsin. These services are not covered by Gundersen Lutheran Health Plan. You can choose to receive these services from any Wisconsin provider that will accept your ForwardHealth ID Card.
