Your statements
If you have received health insurance from Gundersen Lutheran Health Plan in the past you may already be familiar with the standard Explanation of Benefits (EOB). When you review your EOB and Cost Share Fund statement it is important to reference both together. The EOB reflects the payment made by your Consumer Involved Medical Plan; and the Cost Share Fund statement reflects funds that were subsequently paid to your medical provider from your available Cost Share Fund balance.
