Gundersen Lutheran Health Plan staff are available to meet with you to discuss the four Senior Preferred (HMO) plan options available. You can also complete an enrollment application during this appointment, if you choose.

Online: You can complete a Senior Preferred enrollment application by accessing this link OR Medicare Beneficiaries may also enroll in a Medicare Advantage Plan like Senior Preferred through the CMS Medicare Online Enrollment Center located at http://www.medicare.gov.
By mail: You can open, print and complete an application. Please note, applications are time sensitive. If you want to talk to a representative about your eligibility for Senior Preferred or the date your coverage would begin, please contact us.

Interpreter services are provided free of charge to you. A customer service representative is available to assist you Monday through Friday from 8 a.m. to 8 p.m. From October 15 through February 14 we are also available to assist you on Saturdays and Sundays from 8 a.m. to 8 p.m.
